I acknowledge that VALIDOR is an independent verification service and that the resulting Sentinel Audit Report provides a point-in-time verification of credentials. I agree to the processing of my data for this purpose in accordance with the VALIDOR Privacy Policy.*

I understand that this fee covers the manual audit process. While VALIDOR makes every effort to verify, if a practitioner refuses to cooperate or provide consent, the final report will state "Unable to Verify." No refunds are issued for non-cooperation
